The Mobility Parking Scheme (MPS) medical certificate

When applying for a new or renewal MPS card, the medical certificate on the application form must be completed and signed by a medical doctor. Your doctor must also record their provider number on the form.

Note: A person with a Clinically Recognisable Disability (CRD)  that is recorded by the RTA does not need a fresh medical certificate to renew their MPS card. Click the link to the right for more information.

Doctors are aware that the fraudulent use of MPS cards disadvantages the people the scheme is designed to help. Your doctor will not sign the medical certificate unless you genuinely meet the requirements.
