From 18 May 2009, Mobility Parking Scheme applicants who hold a NSW driver licence will also have their medical fitness to drive assessed if the RTA has not been notified of any medical condition on their licence.
The Mobility Parking Scheme application form has been expanded to include a medical review section so that the applicant's medical fitness to drive can be assessed as appropriate.
Driver licence applicants who already hold a Mobility Parking Scheme card may be asked to provide a medical report about their fitness to drive at their next licence application (reissue, renewal or replacement) if the RTA has not been notified of any medical condition on their licences.
Drivers who are applying for a NSW driver licence for the first time and already hold a Mobility Parking Scheme card will be required to provide evidence of their medical fitness to drive before a licence will be issued.
The new expanded Mobility Parking Scheme application forms are available from any motor registry and can be downloaded from the box below.
Why am I being asked to provide a medical report?
The RTA has a responsibility to ensure that all NSW licence holders are medically fit to drive.
We have introduced the medical report requirement to prove a drivers’ fitness to drive when a licence holder also holds a Mobility Parking Scheme card. Safer drivers make safer roads.
Why am I being asked to provide a medical report if my condition is temporary?
With immediate effect, the medical report regarding fitness to drive only needs to be completed for temporary card applicants if you are applying for a further temporary card that will result in you continuously holding a temporary Mobility Parking Scheme Card for more than six consecutive months.
How long has this requirement been in place?
The Mobility Parking Scheme ‘fitness to drive’ medical requirement was introduced on 18 May 2009.
Who is required to provide a medical report?
- Any applicant for the issue, reissue, renewal or replacement of a NSW licence (including interstate and overseas licence transfers) who already has a Mobility Parking Scheme card and is not already subject to medical review by the RTA.
- Any applicant for the issue or renewal of a Mobility Parking Scheme card who holds a NSW licence and is not already subject to medical review by the RTA or whose licence has not been endorsed with the use of a disability aid (‘D’ condition), medical condition ('R' condition) or vehicle modification (‘V’ condition).
Do I have to provide a completed ‘fitness to drive’ medical report first when I am told of the requirement at the motor registry when applying for my MPS card?
No. If you agree, an R019 licence condition is added to your record and a General Medical Assessment Report is printed.
You must return the completed medical report to the RTA to be recorded within eight weeks or your NSW driver licence will be suspended.
If you agree to the issue of the medical report form your Mobility Parking Scheme card can be issued immediately. If you do not agree, the Mobility Parking Scheme card cannot be issued.

What if I do not wish to provide the medical report?
You may return or withdraw your Mobility Parking Scheme card application or renewal, or you may surrender your NSW licence. In either case, the medical report will no longer be required.
