Changing your name
If you have changed your name and you want to change it on your driver licence and vehicle registration you will need to visit a motor registry and complete a Replacement Application form (Roads and Maritime Services (replacing Roads and Traffic Authority) form 1018, available to download from the box at the bottom of this page).
You will also need to produce the original of one of the following documents:
- A marriage certificate # (Commemorative Certificates are not accepted).
- A divorce decree (bearing the name being reverted to).
- A deed poll * registered with the relevant authority.
- A change of name certificate issued by the Registry of Births, Deaths and Marriages.
- A birth certificate showing the name at birth and the new name (Commemorative Certificates are not acceptable).
- A document showing evidence of change of name registered in the Land Titles Office.
In addition to an acceptable change of name document, you must provide a Full proof of identity (POI) document showing the 'original' name; or a List 1 POI document showing the 'original' name and a List 2 POI document showing the 'original' or 'new' name.
Your identity and change of name documents should show a clear link between your names.
Note: Documents not in English must be accompanied by an acceptable translation. See Proof of identity for more information on translations.
# For NSW Marriage Certificates, Roads and Maritime Services (replacing Roads and Traffic Authority) only accepts those issued by the NSW Registry of Births, Deaths and Marriages to support a change of name. You can get the certificate either by calling the Registry of Births, Deaths and Marriages on 1300 655 236 or visiting their website. When applying for a replacement licence, NSW Marriage Certificates issued by churches or celebrants will also be accepted where the married name is already on Roads and Maritime Services (replacing Roads and Traffic Authority) records.
If you were married outside of NSW, a Marriage Certificate from a celebrant or church is also acceptable.
* Since 1 April 1996, the NSW Registry of Births, Deaths and Marriages (BDM) has assumed responsibility for registering all name changes in NSW. To provide evidence of a name change made after 1 April 1996, a Change of Name Certificate from BDM is required. The NSW Land Titles Office no longer registers Deeds Poll or Instruments Evidencing Change of Name.

Changing your address
If you change your address, you must notify Roads and Maritime Services (replacing Roads and Traffic Authority) within 14 days. You can notify your change of address by:
- Attending a motor registry.
- Phoning Roads and Maritime Services (replacing Roads and Traffic Authority) Contact Centre on 13 22 13 or by emailing Customer_Service_Centre@rta.nsw.gov.au.
- By post.
- Online at myRTA.

If you have changed your gender and you want to change it with Roads and Maritime Services, you will need to visit a motor registry and complete an Adjustment of Records (form 1021) if you hold a driver licence or, a NSW Photo Card Application (form 1359) if you hold a NSW Photo Card. Forms are available to download from the box at the bottom of this page).
Along with the completed form and any proof of identity documents you need to provide, you will also need to provide one of the following documents:
- A Birth Certificate showing your new gender, or
- A primary (List 1) POI document showing your gender, or
- A medical certificate from either the surgeon/clinic where the gender reassignment surgery was conducted, or an Australian registered Medical Practitioner that confirms your gender.
