Replacement Traffic Control Cards

Persons who have misplaced their Traffic Control card/s can obtain a replacement from the RTA’s Traffic Control Team by following these steps:

  1. Complete a Statutory Declaration (available from Australia Post or here), with the following details:
  • your name, address and date of birth
  • your card number, training date and training provider (if possible)
  • the reason why you need a replacement card (what happened to the original?)
  • your signature (witnessed by a Justice of the Peace)
  • the Justice of the Peace’s signature and JP number.
  1. Send the Statutory Declaration, with a cheque or money order for $25.00 (per card) made out to the Roads and Traffic Authority (not ‘RTA’) to:

  1. Your card will be reprinted and sent back to you by mail.

To convert your Grey - Traffic Controller Card to the Blue - Traffic Controller Card

Persons holding a Grey Traffic Controller – Non Construction Photo Card and who have completed their NSW WorkCover OHS General Construction Induction training or recognised equivalent can now convert their card from Grey to Blue.

To convert your card:

  • Copy both sides or your NSW WorkCover OHS General Construction Induction Card or your current NSW WorkCover Statement of Training (SOT) Certificate and have it certified by a NSW Justice of the Peace.
  • Arrange a Cheque or Money Order for $25.00 made payable to Roads and Traffic Authority
  • Send your certified copy, payment and a covering letter with your current postal address details and contact number AND your Grey Traffic Controllers – Non Construction Card to:

Your training records will be updated and a Blue Traffic Controllers Photo Card will be issued with the existing expiry date.
