Replacement Traffic Control Cards

Persons who have misplaced their current Traffic Control Photo Card/s can obtain a replacement from Roads and Maritime Services Traffic Control Training Unit by following these steps:

  1. Complete a NSW Statutory Declaration (available from Australia post or here), including the following details:
    • your name, address and date of birth
    • Which Traffic Control Photo Card you need replaced
    • your Traffic Control Photo Card number, training date and training provider (if known)
    • the reason you need a replacement
    • your signature (witnessed by a Justice of the Peace)
    • the Justice of the Peace's signature, JP number and details
  2. Arrange a Cheque or Money Order for $25.00 per card made payable to Roads and Maritime Services.
  3. Send the Statutory Declaration with a cheque or money order to:
    Traffic Control Training Unit
    Roads and Maritime Services
    PO Box 3035
    PARRAMATTA NSW 2124

Note: Expired cards cannot be replaced. You must attend training with an Roads and Maritime Services Licenced training provider.

To convert your Grey Traffic Controller card to a Blue Traffic Controller Card

If you hold a current Traffic Controller – Non Construction Grey Card and you have completed the NSW Work Cover OHS Construction Induction training or recognised equivalent, you can now convert your card to a Traffic Controller Blue card by following these steps

  1. Send a covering letter with your name and address details requesting a conversion of your Grey card to Blue.
  2. Copy both sides of your OHS Construction Induction card or Work Cover NSW Statement of Training Certificate and have it certified by a Justice of the Peace.
  3. Organise a Cheque or Money Order for $25.00 made payable to Roads and Maritime Services (not Roads and Maritime Services or Traffic Control Training)
  4. Send the covering letter, the certified copy of your Construction Induction training, your original Grey Card and payment to:

PO Box 3035
Parramatta NSW 2124

Your training records will be updated and a Blue Traffic Controller Photo Card will be issued with the existing expiry date.

Note: Expired card cannot be converted. You must attend training with an Roads and Maritime Services Licensed training provider

If you are applying for a Replacement and/or Conversion due to a change of name you will also need to do the additional steps below:

  • Enclose a copy of your Change of Name certificate that has been certified by a Justice of the Peace.
  • Your signature on a blank white piece of paper in Black Pen (please note that if the signature is in Blue pen, a photocopy or is on lined paper it will be rejected)
  • Return your original card
